30 May 2024Royal Medical Center

The uterus is a pear-shaped organ of a female that protects and nourishes the fetus during pregnancy. In many instances, the uterus is dilated which means that the size of the uterus is increased as compared to the normal uterus this type of uterus is known as a bulky uterus.

What are the Reasons Responsible for a Bulky Uterus?

Pregnancy is the most common cause of a bulky uterus. However, some other causes are endometrial hyperplasia, polycystic ovarian syndrome, adenomyosis, and fibroids noncancerous tumors that form in the uterine muscle wall. Hormonal imbalances or congenital abnormalities can also cause an enlarged uterus. It's essential to consult a doctor for proper evaluation and treatment.

What is a Bulky Uterus?

An oversized uterus that is bigger than the usual size of a uterus is referred to as a "bulky uterus." Many uterine disorders may be the cause of this hypertrophy. The uterus may enlarge for a variety of reasons, some of which are benign (non-cancerous) and others of which are malignant.

This particular type of disorder could be identified by radiological diagnostic tests such as MRI, ultrasound, or pelvic exam. A bulky uterus cannot be diagnosed easily by radiological diagnosis so we need to examine the signs and symptoms of the patient carefully to give the diagnosis of a bulky uterus.

What are the Main Causes of a Bulky Uterus?

  • Fibroids: The most common reason for a bulky uterus is fibroids. These are non-cancerous tumors called fibroids that develop in the uterine muscular wall which result in pelvic discomfort, severe bleeding, as well as stress on the intestines or in the bladder.

  • Adenomyosis: Growth of endometrial tissue is done in adenomyosis, which typically makes lining in the interior part of the uterus and its muscular wall that results in cramps, pelvic discomfort along severe bleeding.

  • PCOS or polycystic ovarian syndrome: Hormonal imbalance occurs due to weight gain, excessive hair growth, and irregular periods, among other symptoms. It also leads to the thickening of the uterine lining which is the main reason for bulkier appearances occurring in bulky uterus.

  • Endometrial hyperplasia: Non-cancerous thickening of the uterine wall lining is termed Hyperplasia that leads to irregular menstruation or severe bleeding.

  • Endometrial cancer: The most significant cause of a large uterus is endometrial cancer showcasing symptoms such as irregular bleeding, pelvic discomfort, or vaginal discharge. Consult your gynaecologist immediately in case you observe such symptoms.

  • Pregnancy: the main reason for having a bulk uterus is pregnancy, throughout the process of pregnancy the lining of the uterus expands as the uterus provides room for the growing baby.

  • PID or Pelvic Inflammatory Disease: The uterus may expand due to PID's chronic inflammation.

Symptoms of the Bulky Uterus

Although an enlarged uterus may not exhibit any symptoms in and of itself, the underlying cause of the growth frequently does. The following are some typical signs of a bulky uterus along with their potential causes:

  • Abnormal vaginal bleeding could occur with heavy or extended menstruation or irregular bleeding also can occur during or after the menopause

  • Regular removal of the uterine lining could be interfered with by a bulky uterus leading to greater or frequent bleeding

  • Pelvic pain could lead to discomfort, pressure, or cramps in the pelvic or lower abdomen where the pain is due to a bulky uterus which results in extensive strain on nearby organs and nerves. If any individual develops fibroid and adenomyosis they may also undergo pelvic discomfort.

  • Pressure on the rectum or bladder: This can cause constipation, difficulty urinating, frequent urination, or a sensation that the bowels have not been completely emptied. As the uterus expands, it may put pressure on the urinary tract and rectum, causing problems for them to operate normally.

  • Pain during intercourse: This may be brought on by pelvic nerves or pressure on the cervix. Having a bulky uterus leads to discomfort as during intercourse there is a constant shift of cervix and uterine positions.

  • Bloating or swelling in the abdomen: It also occurs due to a bulky uterus due to accumulation of the fluid in the pelvis region. his may be brought on by an enlarged uterus or by a buildup of fluid in the pelvis.

Diagnostic Test for Bulky Uterus

  • A pelvic examination is done to examine any abnormality in the size, shape, or location of the uterus.

  • Imaging tests such as ultrasound, MRI, or CT scan to examine the uterus and find the possible reasons for the formation of the fibroids or adenomyosis.

  • Blood tests are done to examine abnormal levels and hormonal abnormalities

  • Biopsy is done & provides a biopsy of the lining of the uterus to detect any kind of abnormalities

Treatment Option for Bulky Uterus

Progesterone and estrogen-containing birth control tablets could help in the reduction of excessive bleeding and menstrual cycle regulation. GnRH agonists are medications that reduce fibroids by temporarily blocking the synthesis of hormones from ovaries. These drugs work by controlling hormones to reduce hemorrhage and other symptoms by shrinking endometrial tissue or fibroids that are potentially causing the bladder to expand.

Procedures Used in Bulky Uterus

When the fibroid supply of blood is cut off during embolization of the uterine artery, the tumor size shrinks then a thin catheter is inserted into the uterine arteries, and small particles are administered to block the flow of the blood also another surgery is performed called myomectomy is a type of surgery used for eliminating uterine fibroids performed by cutting off their blood supply, fibroids become immobile along with constriction which reduces their potential to alter the size as well as symptoms of the uterus.

It is performed by the procedure termed laparoscopically which involves small incisions on the tissues and abdominally involves a larger incision. Myomectomy is the procedure that removes the fibroid tissue directly leading to the shrinkage of the uterus and thus reducing pain. Studies have shown that it is not favorable for everyone as it depends upon the size and location of the fibroid.

Lifestyle Modification Done for Bulky Uterus

Lifestyle modification is the type in which the chances of the formation of the bulky uterus could be decreased gradually by doing simple normal daily routine modifications.

  • Nutrition: Eating healthy food such as fruits, vegetables, and whole grains should be the priority as they are high in nutrients & fiber which altogether leads to balance in the levels of hormones that altogether decrease inflammation associated with fibroids which is the major cause of bulky uterus.

  • Regulating your Weight: Keep a close eye on your weight as being overweight could lead to symptoms such as excessive bleeding, disbalance in the hormonal levels, and left stains on the bladder.

  • Exercise: In a day try making an effort to do a minimum 30 minutes exercise in a sequence of moderate-to-intense sequence pattern for approximately 5-6 days per week.

  • Stress Reduction: you can practice stress-reduction strategies such as Hormonal imbalances that might worsen due to prolonged stress.

  • Get a Good Night's Sleep: put effort into getting enough sleep and make it a priority to aim for seven to eight hours so that you get a good sleep every night to maintain hormonal balance and general health, getting enough sleep is essential.
